NEW PATIENT FORMS
Before you arrive at Partner in Health for your first visit, please bring the following items:
- Photo ID
- Insurance Card (if applicable)
- Any diagnostic imaging (we prefer the actual images and the report)
- Any Referral Documentation (if applicable)
Your First Visit
On your first visit, you will be given the initial paperwork to fill out about yourself, your current condition, and your past medical history. If you would like, you can download the initial paperwork listed below and bring it with you to your first appointment.

The doctor will then perform a detailed history of your condition. Following the history, the doctor will perform an examination which will include an extensive orthopedic, neurologic, and functional examinations. The doctor may recommend advance imaging if he deems it necessary. Treatment may be initiated on the first visit depending on the exam findings. On the first visit, you may want to allow between 60-90 minutes due to the extensive nature of the initial evaluation and 30 minutes for subsequent visits. Each visit may vary as your symptoms improve and change. We look forward to meeting you!
